Immunization Requirements

Mandatory school vaccines Include DTap (diphtheria, tetanus, pertussis), Hep B (Hepatitis B), MMR (measles, mumps, rubella), Varicella (chickenpox), Polio, and Tdap (tetanus, diphtheria, pertussis). Students entering seventh grade also require the Tdap booster.

Important Notes :

  • Students who are exempt from immunizations for medical or religious reasons must provide a Certificate of Exemption to the school.
  • A religious exemption can be obtained from a county health department.
  • Medical exemptions require a letter from the student’s physician or the official State of Florida Medical Exemption form.
  • The DH 680 form is used to document that a student has met the minimum state immunization requirements.
  • The DH 681 form is used for religious exemptions.

    • Attendance Policy

    Reporting Absences :

    To ensure our students receive the best services possible, attendance is important. Our expectations are that your child be in attendance 95% of the time to serve him/her adequately. If your child is going to be absent, please let us know, if possible, in advance. To ensure your child is excused for their absence, a note is required within 2 days of their return to school.

    Sign-In/Sign-Out :

    To streamline our services we have a sign in/out procedure that parents must follow. ProCare childcare management app can be downloaded to your phone to make it easy for you to sign your student in/out. Because this is our primary way of communication with our families, parents are required to download the app. Any authorized person without access to the app, must come into the office to sign your student in and out (no exceptions). This allows us to communicate more effectively, receive payments, and send out important new blast messages to our families.
